I'll say this one more time. Pain itself if intense enough, can kill you. Why? Because your body enacts a number of mechanisms that respond to it, but can only withstand it for an extremely short time - we're talking tens of seconds. Your heart rate will increase nearly threefold, your lungs will begin to hyperextend to take in more oxygen. these responses are evolved to be able to respond to the pain source and escape it. If you can't? Your body keeps doing this through hormone secretion, and the pain is numbed for a few seconds. Then your hormones run dry and your pain receptors are sending signals so intense to your brain that you begin to shut down.
How? Easy. All singals in your body are electrical impulses. Under so much pain and feedback, your brain fucks up it's computing. Shit begins to act irregular, and vital processes controlled by the Medulla Oblongata likewise begin to fail because your brain is just so overwhelmed. Breathing becomes irregular and shallow, causing a build up of toxic carbon dioxide in your blood etc.
Your heart is strained, it has done so much exertion for no cause and as such, it begins to beat un-rhythmically before it stops altogether. Why? Because it too, relies on the fact that this massive exertion will result in a move away from the pain. But it hasn't, so it's expected to keep beating, but it can't. It's gone over it's limit and simply stops because the cells causing the electrical pulses that causes the beating can no longer sustain the output - they fail.
