My main gripe with the image is that the smudge (or whatever you used to blend) is really obvious in some places, like on the teeth, upper lip and the side of the head, while not so in others. If you were going for smooth photorealism, you really need to slave on every single detail (and you skipped a lot of them, like the wrinkles of his eyes and the grain of his facial hair area) or it'll come out looking awkward. If you weren't, work on making your strokes more defined/painterly and less like the smudge tool. As it is now, it looks sort of like this plastic bastard child of the two.
