If a guy is in the picture he should not be the focus. In these, it's a /h/eterosexual couple doing something together. They're both doing their part. Hence why /u/ is for women being sexual with each other, and /y/ for two men.
/e/ is about the lack of sex, which is why the rules use the word suggestive, not explicit. Sex is sex, regardless of nudity or what body parts are showing. It's also why this wouldn't go on /c/.
