It could be that the glue between the two soles is now dried up and the soles have air between them that makes them make that sound.
Check for breaks in the soles or if its still firmly attached all around.

How exactly is one supposed to wear combat boots? I dig the look when they are sitting on my floor, but putting them on is another story. If I tie them up to the top they look...well...militant. Maybe you can help me! Here are my combat boots. 10.5 just my size. I haven't broken them in completely yet. I usually take 2 hour walks wearing them to try to get them broken in. Usually about an hour into the walk, my right ankle starts killing me. Once I take the boot off, the pain is gone. How can I fix this? Do I need to find different/better boots? Will it go away once the boots are broken in? Am I wearing them wrong?

I don't think the tongue is creasing wrong. I've done my best to make it lay properly and it still happens after a while.
One thing I noticed. The toe cap area is pretty stiff still and when i finish the step, as the sole bends, it feels like the toe cap area is pushing down on my foot, right before the bones in my toes.

There are a few tricks to widen stuff like that, but I bet the toecap has a plastic or fabric with polymer glue cap inside. Those won't be easy to widen without using brute force. Since all widen techniques just widen the leather and not the cap. What you need though is the reinforcment widened that cuts into your foot.
Put your hand inside and try to make a fist. If it's actually moveable I'd wear two pairs of socks for some time, or put a bag of water in them and freeze them.
But all that is shit if the cap inside is unmoveable. You'd have to get a larger size then.

I mostly tie them up until the last three eyelets and wrap it around and tie it in the back. They have some wear but they're in pretty good shape. Buying designer combat boots just feels so homo, I need the real thing. They fit extremely well without the insoles, I couldn't imagine wearing a boot any tighter though I suppose if I was in combat I would need the insoles and thicker socks. People that wear those ridiculously slim boots and bitch about how uncomfortable they are annoy me. Jackboots and other type of boots just aren't for me, although I could go for some motorcycle boots. It's not like I care if I own a motorcycle or not, I'm not in the army either.

Ok, so here's a couple things you got going on.
1.) this might sound stupid, but crush the toe of the boots. It'll soften them up a bit, & with polishing, it'll still look good. It'll flatten out the natural roundness of the toe, but it should also make it more comfortable.
2.) use the speedlace system. ie; what you want to do is when you lace all the way up, instead of lacing the last eyelets, go straight up from the second to last. You know you did it right when you can just pull on the laces & all the eyelets tighten up on you. You really only have to worry about the tongue fit after that if it still pinches.
Sometimes the leather by the ankle is stiff; work the leather with polish & you should be fine. Alternatively, if you want to not crush the toe, send time polishing the entire leather area of the boot. That should soften everything up for you enough to where it should be a lot more comfortable.
