The big qualifier to this is that if someone appears to feel threatened, and then starts to reach for the item, the person's overall demeanor tells volumes. If he appears protective, it's something valuable. If he appears like he's trying to pray the threat out of existence, it's something useless. If he appears ready to respond, it's a weapon of some kind, though still maybe not a gun.

Moreover, a sidearm in a standard holster indicates somebody who regularly carries a sidearm for defensive readiness: somebody who is probably not a threat.
