Hit points represent lesser concussion damage, pain and bleeding, real injuries (from a sprained ankle to being cleaved in two, or having your nervous sytem blown by a spell) are treated seperately. Characters should remember the goblin always has a chance of killing them outright, so players tend to THINK.
Defects : character creation too long if you don't use a spreadsheet, exp system is so retarded I never heard of someone really using it (and anyway goal- and roleplaying-xp are better), exhaustion rules are a bit clunky.
Runequest on the other hand (if d100<skill level, you succeed, basically). I don't know this one as thoroughly, having only played like 10 years ago. Gritty and dangerous too, no HP increase with level. You can play a talking duck, which is awesome (or retarded, but well in some games you can play a lizard with tits, so...).
