So maybe a combination of levels in the waking world where you're a teddy bear brought to life and defending the sleeping target, and levels where you're in the dream world defending him while he completes a task of some sort to overcome an obstacle from his waking life? Escort missions are generally a pain in the ass but they can be good if done right. It would be risky to include such levels, but with a good developer, it could really add to the gameplay variety.
I would think that some small cutscenes in between fights would contribute to the level and monster design

Final level could have you hopping back and forth between the realms to ward off the evils in both. Think light world/ dark world in Shadows of the Damned or Soul Reaver 2, except you'd either have a button for hopping between the realms and you'd be standing in the same place for both. (a screen-in-screen or radar could be used to show the other realm and an icon to display exactly where you'd go if you hopped over)
You can even throw in little sequences of other games like a tower-defense variety or something.
If you succeed in the end, you get a perfectly healthy, confident, happy young man who's off to explore the world. Giving you one last thanks for being there for him before leaving.
Then giving you to his little brother who promises to take care of you, he's been having trouble sleeping lately and could use a new friend.
The game can repeat itself that way. A new Game+ type of thing, but it would have aesthetic changes.
