Make a game with an Infamy and a Fear mechanic. Infamy is awarded whenever you do something that instills fear in people. Once you reach specific levels of infamy, people start fearing you per default, to the point of outright fleeing at your sight.
Lets say somebody annoys you. You choose to execute him in front of everyone, because that's the kind of bad-ass motherfucker you are. Set an example. People start fleeing. The local law-enforcement shows up. You slap their shit. Everybody who watches flinches in fear. You'll get infamous. At the end of the game you'll be some kind of Voldemort-esque villain where people start shaking when somebody even mentions your name. Eventually some great hero shows up and tries to defeat you.
You kill him, marveling at how pathetic he is.Or you're playing a stealthy character. For every guy you manage to kill without being spotted, you are given a certain amount of Infamy. At the end of the game, you're the freaking Batman, Grey Fox, and Solid Snake combined and the enemies start pissing their pants if they know you're in the same room as them. You become a living legend. Nobody believes you even exist due to the outrageous things they hear about you.
Or do a "guns blazing" playthrough. Murder. EVERYONE. Endgame you'll be so powerful that the enemies go NOPE.jpg when they see you and get the fuck out of there. Most of the time you won't even have to fight them, they just straight out flee from you. Kill them while their backs are turned to you. You'll be Darth fucking Vader.
